Insurances Accepted

We have made prior arrangements with many insurance companies and health plans. We will file claims with those plans with whom we have an agreement with. We will collect any required co-payment, co-insurance, or deductible at the time of service. In the event your health plan determines services to be "not covered", you will be responsible for the complete charge. While we may be PARTICIPATING, IN-NETWORK or ACCEPTING the following insurance companies, we may not take every plan that is offered. We do not accept HMO or Medicare Advantage/Replacement plans. Please either contact your insurance company or our office to verify we accept your plan prior to your visit. We are currently in-network with:
